Hello Parents and Children,
This letter is to inform you that I will be retiring on December 31, 2019, and I have selected Children’s Medical Office in North Andover to take over my practice.
First, let me say that is has been a privilege to care for my patients and their families over the more than 30 years that I have been in practice. My patients and their families have shared multiple stories with me over the years, and I have shared many stories of my family. We have shared laughter and sometime tears in this pursuit. While I have advised you along this journey, I have also learned from you. For that, I am forever grateful.
I have chosen Children’s Medical Office to take over my practice for many reasons. Children’s Medical Office has provided high quality pediatric care in our community since 1989. The practice’s three pediatricians and four nurse practitioners include both male and female providers. Because they are part of the Boston Children’s Hospital Network, as I am, they participate with the same insurers that I do, and they use the same medical record system that I do. They have office hours seven days a week, including early morning walk-in hours and early evening appointment hours, and their office is less than two miles from my current location. After having a number of conversations with the Children’s Medical Office providers and observing them providing patient care, I am thoroughly convinced that they will take excellent care of my patients. A welcome letter from Children’s Medical Office is enclosed with this letter.
WHAT DO YOU HAVE TO DO?
My office will continue to schedule both well visits (physicals) and sick visits through December 20, 2019. I will continue to see sick patients through December 26, 2019. We will notify you of any preexisting appointments that you have scheduled after December 31, 2019, so you can reschedule with your new pediatrician.
If you wish to transition your child(ren)’s care to a Children’s Medical Office pediatrician, you need to inform Children’s Medical Office(by calling 978-975-3355) and notify your insurance. Please do this before than December 27, 2019. Because Children’s Medical Office is part of the Boston Children’s Hospital network, Children’s Medical Office will have access to my electronic medical records if you transfer care to one of their providers.
If you wish to transition to a different pediatrician, you will need to call your insurance company to identify a pediatrician that takes your insurance, and you will need to complete the enclosed Medical Release Form and send it back to our office. This will allow us to copy your records and send them to your new pediatrician. We strongly suggest that you do this no later than December 10th, so that we have time to copy your records before our office closes on December 27, 2019. After that date, you may obtain your records (or arrange for them to be transferred) by contacting Children’s Medical Office at 978-975-3355
Please be sure to arrange for a new pediatrician to care for your child(ren) before December 27,2019, as my office will no longer be available to answer questions or provide advice about your child(ren)’s health after we close at 4pm that day.
